Required EValuation Forms
All interns are required to submit a mid-term and final evaluation to remain in good standing.
Failure to turn in these evaluations will impact your standing and status in the program.
Materials can be downloaded below:
- Midterm evaluation is due on or before your 6th month of the internship.
- Final evaluation is due no later than 11 and 1/2 months into your internship.
